Regional Program Coordinator - South Side, Chicago

At First Mile Care we’re building the nation’s first affordable, scalable, and sustainable platform to improve the lives of people who have prediabetes. Our goal is to half the number of people in the U.S. with prediabetes by 2028. How? By scaling the CDC’s proven Diabetes Prevention Program (DPP). We are committed to creating a nationwide network of First Mile Coaches in the vast majority of the U.S.’s 42,000 ZIP codes: local heroes trained to help their fellow community members make the right decisions on a daily basis so they can gain control of their health and reverse prediabetes. While this position is from your remote location, we are only accepting applicants based in the South Side, Chicago Area.

 

The Position

The Regional Program Coordinator will work closely with the Director of Operations on key needs of the DPP program as it relates to the sessions and coordination of program schedules and coaching assignments.  The Regional Program Coordinator must have experience as a health coach and will have the opportunity to coach groups. They will be responsible for:

  • Day-to-Day Management of Diabetes Prevention Program Sessions. Including but not limited to:
    • Identifying program sites and managing location rental contracts with local program sites.
    • Coordinating all aspects of participant materials including session reminders and handouts. Managing issues that may arise including relevant participant communications.
    • Organizing substitute coaches or new session dates if a coach is unable to attend their assigned class.
    • Organizing participant events including but not limited to planning, reminders, and execution of virtual and/or in-person events.
    • Manage organization and delivery of session 1 for new cohorts. This includes managing the participant intake process, confirming the details of session 1, and following up with coaches and participants in the event that dates or locations change.
    • Provide periodic reports to communicate key indicators of performance.

 

  • Coach Support – Act as key contact for new and existing local coaches.
    • Assist the Director of Operations to coordinate coach hiring, onboarding, training and support processes. Review coach performance as necessary. Maintain procedure manuals; ensure appropriate communication to staff of changes in procedures.
    • Proactively manage program data entry by ensuring Coaches enter program data into the appropriate systems in an accurate and timely manner (within 2 business days of a session). Suggest and implement program related data system solutions.
  • Act as liaison between coaches and the operations team to communicate any concerns.
  • Proactively manage current regional protocols such as local safety and public health recommendations.
  • DPP Coaching – Facilitate weekly or bi-weekly sessions with a group of 8-15 participants through the year-long Diabetes Prevention Program.

 

Required Qualifications

  • 2+ years of experience in health education and/or coaching with groups of individuals.
  • Familiarity and proficiency with motivational interviewing and group facilitation.
  • Be committed to and supportive of the First Mile Care mission.
  • Have strong communication skills both written and oral. Detail oriented and highly organized.
  • Strong computer skills including proficiency in Microsoft Excel.
  • Excellent problem-solving skills. Ability to improvise and adapt in any situation. Ability to work on multiple projects with tight time constraints.
  • Located in or near South Side, Chicago, IL. Knowledgeable of the South Side neighborhoods.

 

Preferred Qualifications

  • Bachelor’s degree and/or combination of Associate’s Degree and work experience
  • Minimum of 2 years experience in a healthcare capacity with 1 year experience in public health, social service, customer service, and/or healthcare industry.
  • Experience managing health coaching relationships between coaches and participants.
  • Experience scouting locations for community-based events.

 

Job Type: Full Time, Employee

Salary: $45,000-$55,000 per year, healthcare benefits, HSA available

The position is remote. Depending on location, the employee may have the opportunity to meet with team members onsite. Candidates must be authorized to work lawfully in the United States.
