Development is already underway for First Mile Care’s initial platform offering which enables community-based peer-to-peer connection for chronic conditions. The platform will give people living with prediabetes – higher-than-normal blood sugar level but not yet Type 2 diabetes – the ongoing support they need to make lifestyle decisions that can reverse prediabetes and reduce the risk that their condition will develop into type 2 diabetes.

The program recognizes that if lifestyle changes are to be successful, coaching needs to be easy and consistent. This ongoing support provides individuals with the support they need toreduce the risk that their condition will develop into type 2 diabetes. First Mile Care will provide access to certified coaches nearby at an affordable price that is often completely covered by health insurance.

With $2 million in seed funding, a seasoned founding team, and significant market interest in its approach to combatting chronic disease, First Mile Care is embarking on a mission to cut in half the number of people in the U.S. with prediabetes by 2028. Medicare reimbursement for the National DPP began in April 2018, making it affordable for millions of people with prediabetes to access the preventive care they need, while a growing number of private health plans are also including CDC-recognized programs in health benefits as well.
